HSHS St. Anthony’s Memorial Hospital to host Community Connected Care Collaborative Program
(Effingham, IL) – The Community Connected Care Collaborative, in partnership with HSHS St. Anthony’s Memorial Hospital, invites members of the greater Effingham community to join them for a public program on Thursday, March 28 on health care topics that matter to you. This program will be held from 5 to 7 p.m. at the Effingham Public Library.
The event will include a presentation on Hospitalist Services by Dr. Andrew Mahtani, Vituity hospitalist located at St. Anthony’s Memorial Hospital. Additionally, St. Anthony’s Pharmacist Kris Keplar, PharmD, will discuss Medication Management and Antibiotic Use. The event will also feature informational booths and refreshments.
The Community Connected Care Collaborative is a joint venture between HSHS St. Anthony’s Memorial Hospital and nursing facilities, physician’s offices, and community agencies in the greater Effingham area. The purpose of the Collaborative is to discuss opportunities and implement strategies which enhance patient care across the health care continuum.
