SBL Providing Physicals to Mattoon Athletes
Sarah Bush Lincoln will provide sports physicals to athletes from Mattoon schools.
Sports physical clinics will be held from 5pm to 7pm on Wednesday, June 29, at the SBL Family Medical Center, 200 Richmond Ave. East, Mattoon. The cost is $20 per physical with all fees being donated to the Mattoon Athletic Booster Club. Appointments are required and can be made by calling 217 234-7000 (opt. 1 & 2).
Immunizations will not be given at either clinic. Cash or check only will be accepted. Insurance will not be billed, nor will insurance co-pays be collected. Parent or legal guardian must sign a medical history form or be in attendance.
